[10/23/2026] Employee CSV Upload issue — From approximately 4:45 PM PDT on October 22 until 3:10 AM PDT on October 23, 2026, an issue occurred where employee lists uploaded via CSV or automated sync scripts, such as Active Directory, were not processed correctly. During this window, a routine security update to a software library caused the system to misread every uploaded file as empty, which the platform interpreted as a request to remove employees from the directory. This resulted in missing employee records, deactivated user accounts, and the loss of desk assignments and location roles. Our engineering team identified the cause as a change in how the updated library saves downloaded files, which was unfortunately not caught during release testing. We have since rolled back the update, deployed a permanent fix, and performed a data backfill from a previous database snapshot to restore employee records and recreate lost desk assignments.  An internal post-mortem has been scheduled to analyze the incident and note key learnings and action items.
